DepEd Order No.11, s. 2018 is the guidelines on the preparation and checking of school forms for the preparation, updating and evaluation of school forms in basic education starting end of the school year 2017-2018.
In line with DepEd Order No. 11 s. 2018 Ma’am Jeanete Kiong, Division Planning Officer II; Mr. Melchor C. Tican, Public School District Supervisor of La Trinidad District; Mr. Marcelino S. Baldo, principal of La Trinidad Central School; Mr. Mark Binay-an, school head of Alno Kadorie Elementary School; with BES principal Mr. Franklyn Fianza, checked the forms of Kindergarten, Grade I and Grade VI of Balili Elementary School last March 27, 2018. They emphasized that SF1, SF2, SF3, SF4, SF5, SF5K, SF6, SF10 and generated LIS should jibe with correct spelling of names, birth certificate from PSA and LRN to ensure consistency and quality of school forms.
To those forms of other grades which were not yet checked, Mr. Franklyn B. Fianza formed school checking committee who will check the forms. The committee composed of the ICT coordinator Mrs. Efagenia Paing, LIS coordinator Mrs. Elmerine Panagan, Mrs. Angeline Viado, master teacher 2; Mrs. Aurea  Valdez, master teacher 1; with the help of Miss Ailyn Simeon and  Elializa Dalisen of Balili Elementary School La Trinidad, Benguet. HERMANA P.
